Understand payment before treatment.
We participate with almost all insurance plans except AHCCCS. Some HMO plans are accepted. Please call our office to verify your coverage. CareCredit and Cherry financing are available, subject to the providers’ terms and approval.

How should I return my patient registration form?
You can download the blank registration form before your visit. Contact the office for an approved way to return it, or bring it to your appointment. Do not email or upload a completed medical or registration form through this website.
Do you accept my dental insurance?
We participate with almost all insurance plans except AHCCCS. Some HMO plans are accepted. Please call our office to verify your coverage. Accepting insurance does not necessarily mean we are in-network with your plan. We can help you check before treatment.
